    Benefits of Membership    

Members in Good Standing with the Association are able to use the Association's Seal in their businesses indicating their commitment to the Association's high Standards of Service and Business Ethics. We invite you to completely review our Web site. Check out the Member Benefits page.

Membership in the Association is voluntary and all non-medical home care business organizations, and those affiliated with the industry, are invited to join the Association.

Any sole proprietorships, partnerships, corporations, companies, associations, or other form of business endeavor of good character and outstanding business and professional reputation engaged in the non-medical home care industry in Arizona may be Business Members. Please choose the category for which you are applying for membership.

  • Business Members - Those businesses, sole proprietorships, partnerships, corporations, companies, associations, or other forms of business endeavor of good character who are actively engaged providing non-medical home care services. Business members are voting members of the Association.

  • Associate Members - Those businesses that are engaged in the non-medical home care industry as a part of providing other, medical, palliative, or other home care services. These Members will not be voting members of the Association; except that any Associate Member elected to the Board of Directors will have voting privileges at any meeting of the Board of Directors. Examples are:
              (1) Licensed Home Health Care Agencies;
              (2) Hospice Agencies; Insurance Companies;
              (3) Senior Specific Services...

     The process of becoming a member of the Arizona Non-Medical Home Care Association is not difficult. There is an application that is required to be completed by each business entity. In addition to the application, there is a Credentialing Standards Agreement that is required to be signed by a designated representative of each prospective member-business. These forms are available on the "Join or Renew" page and are in PDF format. Any interested business should download these documents, print & complete them. If help is needed completing the documents, or there are questions, please call the Association at 602-283-3503 for assistance. When the forms are completed, they should be mailed to the Arizona Non-Medical Home Care Association, PO Box 8042 Tempe, Arizona  85281.

     Upon receipt of a complete Application Packet, the Application will be processed. This process includes verification of all the information on the completed Application. The verification process could include an on-site visit to the Applicant business, to verify items on the Application. After the Application is verified and the Applicant Business has been confirmed to be verified and eligible, the Board of Directors reviews all the information and votes the Applicant Business into the Association. Upon confirmation of membership, the new Member will be sent a Certificate of Membership in the Association.  As a Member in Good Standing, the new Member will have access to use the Association's Seal and all other Member rights.
